Insurance Benefits
As a courtesy to our patients, we attempt to verify one's insurance benefits prior to their first visit with us. However, it is your responsibility to know your health plan benefits to ensure that we were given the correct information by your insurance company. Even though Strive representatives verify insurance ahead of time, there is no guarantee of coverage or payment. This is only determined after the claim for each date of service has been processed by your insurance company.
Payment Policy
Medical supplies recommended by the physical therapists for facilitation of treatment are purchased at the patient's discretion and are not submitted to the insurance company. These items will be paid for in full at the time they are received by the patient.
Any applicable deductible or co-payment will be collected for each office visit. We accept cash, personal check and all major credit cards.
If the insurance explanation of benefits (EOB's) indicates that a patient has overpaid, they will be reimbursed accordingly.
